Kids Path is a unique program for children, teens and their families, coping with a serious illness. Kids Path’s team offers a variety of supportive care models with a goal of enhancing the quality of life for children, teens and those who share their lives.
Kids Path staff work closely with the child’s family and physician to coordinate care. Here are some of the ways we can help:
Our Kids Path clinical staff offers a team approach to care for children with life-limiting conditions. These children usually have a life expectancy of months rather than years. A physician’s order is required for hospice care and may include a variety of services, such as nursing care, counseling and volunteer services for all family members.
In addition to a Kids Path registered nurse (RN), additional support may be provided by:
- Counselors (for patient, family, siblings).
- Hospice aide.
- Social workers.

CAP/C Case Management
For patients who are enrolled in Community Alternatives Program for Children (CAP/C), our Kids Path nurses serve as the case manager and assist with identifying care needs. Our case manager will develop a plan of care to meet your child’s needs, monitor, and coordinate the services and supplies in that plan of care through the CAP/C program.
We offer supportive services for parents who are expecting a baby with a life-threatening condition at, or shortly after, birth.

Other Medical Resources
In 2017, Kids Path entered into a partnership with Cone Health and Advanced Home Care to offer a model of community-based pediatric palliative care. Cone Health’s Palliative Medicine Team and Pediatric Specialty Clinic coordinates care for these patients across care settings. Advanced Home Care provides home health nursing services. Kids Path provides wrap-around services, including counseling, chaplain support, and volunteer transportation to and from counseling sessions.
