How to Make A Referral

Making a patient referral to HPCG:

• Telephone referrals are accepted seven (7) days a week, twenty-four (24) hours a day:
           o During Business Hours: Call our Referral Center at 336.621.7575.
           o After Business Hours: Call our HPCG on-call number 336.621.8800.

• Fax referrals to 336.478.2541 (see form below):
           o Note: Faxed referrals are only processed Monday - Friday, 8:30 am - 5:00 pm.
